• Receive information about your diagnosis, treatment options, risks, and expected outcomes in a manner you can understand.

• Know the name and role of individuals involved in your care.
• Receive information about pain management and have your pain addressed appropriately.
• Review, obtain copies of, and request amendments to your medical records as permitted by law.

• Be informed if your physician or the facility has a financial interest in services or equipment used in your care, when applicable.

Patients also play an important role in ensuring safe and effective care. As a patient, you are responsible for:
• Providing accurate and complete information about your health history, medications, allergies, and current condition.
• Informing your healthcare provider of any changes in your condition.
• Following treatment plans and instructions provided by your healthcare team.
• Asking questions if you do not understand instructions or information about your care.
• Keeping scheduled appointments and notifying the facility if you must cancel or reschedule.
• Following facility rules and respecting staff, other patients, and property.
• Arranging for a responsible adult to transport you home and assist you after surgery when required.
• Meeting financial obligations for services provided and working with the facility regarding billing concerns.
• Respecting the rights and privacy of other patients and staff.
Patients are encouraged to share concerns or complaints with facility staff so issues can be addressed promptly. Concerns may also be submitted in writing to facility administration.
